Passed away peacefully after a long illness on Monday November 1st, 2010 at Waipuna Hospice.

Dearly loved wife of Ted. Much loved Mum of Shirley and Michael. Cherished Nan of Michaela and Scott.

The memorial service for Hazel will be held at the Katikati Baptist Church, 2
Wedgewood Street, on Thursday November 4th, at 1.00pm.

In lieu of flowers donations to Waipuna Hospice, P O Box 1156, Tauranga would be appreciated and may be left at the service.
